For those who would like to enter a team, individual or initiative for one of the categories of the 2018 Ophthalmology Honours you simply need to complete an entry form as described below. Alternatively, if you would like a particular team/individual to be recognised for their exceptional work in ophthalmology, but don't feel in a position to submit a full entry, please complete the simple nomination form below and a letter will be sent to the nominated department director, notifying them of the nomination and inviting them to submit and facilitate a full entry.

How do I submit an entry form?

Entering is easy, simply download an entry form, complete the relevant sections, and submit by either post or email:

How do I nominate a team/individual?

To nominate an individual or team for their exceptional work please complete the form below giving your clear rationale for why they should be recognised.

Please note, if you decide to make a nomination, you agree to receive email communication relating to the Ophthalmology Honours programme. Health Unlimited will share your name, hospital name and nomination with Bayer for the purposes of providing updates to the programme.

Health Unlimited are responsible for holding your data for these purposes. Your data will be held in accordance with the Data Protection Act. If you are a finalist, information on your entry will be saved and used with your express permission in future Ophthalmology Honours materials and on the website for the purposes of sharing best practice and highlight previous successful entries. In all other circumstances your data will be deleted one year after the Honours presentation has taken place. All data will only be used in relation to the Ophthalmology Honours programme.
